Rights and Responsibilities

Every time you visit a hospital, physician or other healthcare provider a record of your visit is made. Typically, this record contains your symptoms, examination and test results, diagnoses, treatment, a plan for future care or treatment, and billing-related information.

Patient Privacy

Patient Privacy rights apply to all records of your care generated by the hospital, whether made by hospital personnel, agents of the hospital, or your personal doctor. If your doctor/ medical specialist is not a member of our hospital staff he may have different policies or notices regarding use and disclosure of your medical information created in his office or clinic.

Our Responsibilities

SMMC maintains the privacy of your health information in strictest confidence. Your medical history and family’s personal information will be kept confidential and except for the purposes listed below, our staff will not disclose or share your medical information with others outside our facility except under written direction from the patient.

Your Health Information Rights

Although your health record of your visits and services received at our hospital is the physical property of our facility, you have the right to inspect and obtain a copy of the medical information logged in your patient file. You also have the right to instruct us to release your medical information to third parties, however, written instructions together with the payment of a fee is required to do so.
